Mainly because you’re standing very close to the ball and lining it up on the heel of the club at address I would say

Look at the space between your lower body and your hands at setup. Then compare it to impact.
Change either of those two (space between em at setup or at impact).

Club is getting pulled behind your chest in the backswing causing early extension otw down. Get away from the ball a bit and feel like you’re making a wider arc throughout the entire swing with the clubhead.

Looks to me like you stall out your hips on the way down and throw the club out at the ball. Think about pushing off the ground and toward the ball with your left toes. That will get your left hip back.
The other part of this is hitting the ball on your right side, not so much in front of you.
You will likely be able to sequence this better if you get into your left leg sooner and don’t go so deep on your backswing (especially with a wedge).
1. Shorter turn, planting and pushing off left foot sooner. 2. Hips keep rotating, set in motion by your left foot pushing into the ground and toward the ball. 3. Hit the ball on your trail side.
