Your flipping your hands at impact, and also taking your arms way to far back. You can see your hips shift left in your backswing especially near the top. Feel like you only rotate half of that amount and keep your wrist angles.
Keep your head still. You shouldn’t be straining to keep your eye on the ball. Let your wrists break a bit here so you can point your club toward your target.

I’d start with shortening your swing.
Then find out why you stand up out of your posture.

I guess the key question is where do you lose shots? If you keep the ball in play and advance it a decent distance, maybe you should not mess with your swing but work on your wedges and putting? I a 19 hcp as well and I know that I lose shots within 75 m of the pin.

You’ve got a reverse pivot in your backswing, are reaching with your arms after your body can’t turn any more, that forces your head up creating an up and down motion, your right elbow is too far away from you body at the top of your backswing and your balance is poor as evidenced by your right foot leaving the ground before you even make impact. There is no single tip that is going to fix all of these errors other than to get and instructor for a couple of lessons to rebuild your swing from the ground up because it’s a total mess. I can’t see you getting your HC down until you make major changes to your entire swing.
